Aged care questions, answered.
What aged care services do you provide?
Centre of Hope provides in-home and community-based supports including personal care, daily living assistance, companionship, community access, transport and appointment support, assistance with personal affairs, medication assistance, continence support, care coordination and budgeting support.
Do you provide care coordination?
Yes. Where included in the service arrangement, we can help organise services, communicate with relevant providers, coordinate appointments and support continuity of care. The client remains involved in decisions about their services and preferences.
Can you help me manage my service budget?
Yes. Where budgeting support forms part of the service arrangement, we can help you understand service costs, plan agreed supports within the available budget and keep track of planned service use. We provide information to support your decisions rather than taking control of your personal finances.
Which areas of Sydney do you service?
We provide services across all Sydney areas. Service commencement and worker matching will depend on the person's location, required support times and worker availability.
Can I choose my support worker?
Yes. We value choice and will work with you to understand your preferences and find a suitable support worker, subject to worker availability.
Can a worker take me to appointments?
Yes. Workers can support you to attend medical, allied health and other appointments, as well as community activities, shopping and everyday errands. Transport arrangements and applicable kilometre charges will be discussed beforehand.
Can you assist with personal care?
Yes. We can provide respectful personal care support based on individual needs, including assistance with showering, dressing, grooming, toileting, continence care and other agreed daily living activities.
Can you help with medication?
Yes. Where appropriate and within agreed service arrangements, trained workers can assist with self-administration of medication, provide reminders and support established medication routines.
Do you provide weekend services?
Yes. Services can be arranged on weekdays and weekends, subject to worker availability. Weekend and public holiday rates may differ and will be shown in our price list.
Can you support people from culturally diverse backgrounds?
Yes. Centre of Hope welcomes people from all cultural and language backgrounds. We respect culture, beliefs, language, food preferences, family relationships and individual routines.
How much do your services cost?
We aim to provide quality support at affordable rates. Our aged care price list will clearly show relevant weekday, non-standard, weekend, public holiday, transport and other applicable service charges. Until it is published, we will talk you through the costs of your agreed services before support starts.
How quickly can services start?
We aim to commence services as soon as possible. Once we understand your needs, location, preferred schedule and worker preferences, our team will work to arrange suitable support promptly.
What happens if my regular worker is unavailable?
We maintain a team of support workers so that, where possible, another suitable worker can cover a shift when a regular worker is unavailable. We will communicate any change and aim to maintain continuity of support.
