Book an appointment

A first conversation, on your terms.

Pick a time, tell us a little about your situation, and we will be in touch to confirm. No commitment, no pressure, no NDIS-speak.

Free

No cost to book or attend

The first conversation is on us, regardless of whether you decide to work with our team afterwards.

30 to 45 min

Enough time to listen

Long enough to understand where you are and what you need, short enough to fit around a busy week.

No pressure

Decide in your own time

If we are not the right fit, we will say so and help you find someone who is. Your call, always.

Book your time

Tell us when suits you best.

Fill in the form and we will be in touch to confirm.

Book an appointment
Optional but helpful

What to have handy for the first call.

None of these are required, but having one or two on hand makes the conversation more productive.

1

Your NDIS plan

A PDF or paper copy. If you do not have one yet, that is fine, just let us know.

2

Current providers

A list of any support workers, coaches, or therapists you are already working with.

3

Two or three goals

What you want to work on this year. Big or small, written down or just in your head.

4

Questions

Anything you have always wondered about your plan, your funding, or your supports.

Common questions

Before you book, good to know.

If your question is not here, just call us or include it in the form.

Do I need a referral to book?

No. You can book directly with us as an NDIS participant, family member, or carer. If you are an external organisation referring a client, you can use this form too, or use our dedicated referral form instead.

Is there a cost for the first appointment?

No cost. The first conversation is free regardless of whether you decide to work with us afterwards.

If you choose to start working with Centre of Hope, ongoing sessions are funded through your NDIS plan under Support Coordination, Recovery Coaching, or whichever service fits your situation. We will explain all of this clearly during the first call.

What if I need to cancel or reschedule?

Just let us know as soon as you can, by phone or email, and we will reschedule with no fuss. No cancellation fees for the first appointment.

Can the appointment happen at my home?

Yes, depending on where you live in NSW. We do home visits across Western Sydney and the wider metro area, and we can also meet at a community space that works for you. If a home visit is not practical, phone and video appointments work just as well for the first conversation.

How quickly will I hear back?

Within one business day. If you book on a Friday afternoon or over the weekend, you will hear from us first thing Monday morning.

Booking is the easy part. The work that actually changes things starts when we sit down together.
