00Overview

Centre of Hope provides NDIS positive behaviour support across Sydney and NSW for children, young people and adults with disability who experience behaviours of concern. 01What's included

Behaviour support that starts with why.

Behaviour is often a form of communication. Our behaviour support looks for the reasons behind it and the needs that are not being met, then builds strategies that make everyday life better.

01

Behaviour support assessments

Including a functional behaviour assessment: we look at triggers, the environment, communication needs and the purpose a behaviour may serve, so strategies address causes rather than only the behaviour.

02

Positive behaviour support plans

An individual plan built around your strengths, preferences and goals, focused on quality of life, independence, new skills and proactive, respectful strategies.

03

Children and young people

Support with emotional regulation, communication, social interaction and changes in routine, working with parents, carers, schools and other professionals.

04

Adults

Personalised behaviour support for adults with disability, including people with complex support needs, at home and in the community.

05

Training for families, carers and support workers

Practical guidance and coaching so everyone in your support network understands the strategies and uses them the same way.

06

Reviews and monitoring

We check progress, look at whether strategies are working, and update the behaviour support plan when your needs change.

07

Telehealth where it suits

Some appointments can be held by video, where that works well for you.

Restrictive practices

Behaviour support plans sometimes include restrictive practices. When they do, NDIS rules apply. The plan must be developed by an NDIS behaviour support practitioner, the aim is always to reduce and stop the use of regulated restrictive practices, their use must be authorised, and implementing providers report it to the NDIS Commission.

The NDIS Commission says high risk practices should never be in a behaviour support plan. Read more on the NDIS Commission's behaviour support and restrictive practices page.

02Who it's for

Behaviour support is for people, families and the teams around them.

  • Children and young people who find emotions, communication, social situations or changes hard
  • Adults with disability, including people with complex support needs
  • People whose behaviours of concern affect daily life, relationships, independence or community participation
  • Families, carers, schools and support teams who want consistent, practical strategies
  • Participants with behaviour support funding in the Improved relationships category of their plan
03Our approach

Behaviour is communication.

Behaviours of concern can be shaped by communication difficulties, sensory needs, emotional distress, health conditions, the environment or unmet support needs. Rather than trying to control behaviour, we work out what the person is communicating and find positive ways to meet their needs. Our approach focuses on:

  • Understanding the person and their unique needs
  • Respecting choice, dignity and human rights
  • Building positive communication and coping skills
  • Improving emotional wellbeing and independence
  • Reducing the need for restrictive practices
  • Safe and meaningful participation in the community
  • Building the confidence of families, carers and support teams

Where support happens

Strategies work best where people live, learn and spend their time. Depending on your needs, our practitioners may work with you in your family home, supported independent living (SIL) accommodation, school or early childhood settings, day programs and community settings, or by telehealth, across Sydney.

04How it works

Five steps. At your pace.

Referrals are welcome from participants, families, guardians, support coordinators and providers.

A first conversation

You, your family or your support coordinator call or send an enquiry. We listen to what is happening and what you hope will change.

Check your funding

We look at the behaviour support funding in your plan together, so you know what is available.

Assessment

Our practitioner gets to know you, talks with the people who support you and looks at the settings where things happen.

Your behaviour support plan

We write the plan with you, in plain language, and share it with the people who will use it.

Training and review

We train your support team, keep an eye on progress and review the plan when things change.

05Funding

How the NDIS funds behaviour support

Capacity Building · Improved relationships

Positive behaviour support is usually paid from Improved relationships in your Capacity Building budget. The NDIA says this category can include behaviour support plans and behaviour supports.

The NDIS support items are Specialist Behavioural Intervention Support (11_022_0110_7_3) and Behaviour Management Plan Implementation (11_023_0110_7_3). The Pricing Schedule says these are delivered by providers registered in the NDIS Commission's specialist positive behaviour support registration group, as Centre of Hope is.

Capacity building funding is a stated support, so it cannot be moved between categories. Current price limits are in the NDIS Pricing Schedule. They are maximums, not fixed prices.

06What you can expect

The way we work with you.

Plain language

No jargon. You will understand the assessment, the plan and why each strategy is in it.

Your goals first

Strategies are tied to your goals and quality of life, and we check regularly that they are helping.

Rights and dignity

We respect your choices and your rights, and we work to reduce restrictive practices, never to add them.

Working with your team

We work with families, guardians, schools, support coordinators and the other professionals and providers in your support.

Your choice

You choose your providers. If we also provide another of your supports, we tell you plainly, talk through the alternatives and leave the decision with you.

Regular review

Plans are reviewed and updated as your needs change.

07Questions

Good questions, honest answers.

What is positive behaviour support?

An approach that looks at why a behaviour happens and what the person needs, then builds skills and changes the environment so everyday life gets better. The NDIS Commission says it helps others understand why a person acts a certain way and how to better support them.

Does my NDIS plan need behaviour support funding?

For NDIS-funded behaviour support, yes: it is usually in the Improved relationships category of your Capacity Building budget. If it is not in your plan, talk to your support coordinator, planner or partner about your needs, or call us and we will talk it through.

What is a functional behaviour assessment?

Part of a behaviour support assessment. The practitioner looks at what happens before and after a behaviour, the setting, communication and health, to understand what purpose the behaviour serves. The NDIS Commission publishes a practice guide on it.

What happens if a restrictive practice is needed?

NDIS rules apply. A behaviour support plan that includes a regulated restrictive practice must be developed by an NDIS behaviour support practitioner, its use must be authorised and reported, and the aim is always to reduce and stop it. We explain each step to you and your family.

Do you work with children and schools?

Yes. We support children and young people and work with parents, carers, schools and early childhood services, so strategies are used the same way at home and at school.

Can you train our support workers?

Yes. We provide practical guidance, coaching and training to families, carers, support workers and service providers on the strategies in a behaviour support plan.

Support that starts with understanding.

Tell us what is happening and we will talk through whether behaviour support is the right fit.
