Are You the Right Fit?

We're seeking experienced, values-driven Support Coordinators and Psychosocial Recovery Coaches who share our commitment to person-centered, trauma-informed care.

Qualifications:

  • Relevant tertiary qualification (Social Work, Occupational Therapy, Mental Health, Counseling, Psychology, or allied health)
  • Current NDIS Worker Screening Check
  • ABN and appropriate insurance coverage
  • Working with Children Check
  • First Aid certificate and CPR certification
  • Completed NDIS Workers Orientation Module

 

Experience:

  • Active caseload of NDIS participants ready to transition
  • Demonstrated experience as a Support Coordinator and/or Psychosocial Recovery Coach
  • Strong understanding of trauma-informed care and recovery-oriented practices
  • Experience with complex case management and multi-system coordination
  • Thorough knowledge of NDIS policies, procedures, and funding categories

What You'll Do

As a Support Coordinator or Psychosocial Recovery Coach with Centre of Hope, you’ll provide person-centered support to NDIS participants across NSW, specializing in areas that align with your expertise.

You’ll assist NDIS participants in implementing and maximizing their plans through:

  • Plan Understanding & Implementation — Helping participants understand their NDIS funding and make informed decisions about support choices
  • Service Connection & Coordination — Linking participants with quality providers across therapy, daily living, community participation, and specialist supports
  • Multi-Provider Coordination — Ensuring all services work together cohesively to support participant goals
  • Capacity Building — Empowering participants to develop skills for eventual independent plan management
  • Crisis Support — Providing guidance when issues arise with providers or plan implementation
  • Review Preparation — Tracking progress, gathering evidence, and advocating for appropriate funding at plan reviews
  • Complex Needs Support — Supporting participants navigating multiple systems, including justice, housing, and healthcare

You’ll support participants with psychosocial disabilities on their recovery journey through:

  • Individualized Recovery Planning — Co-designing recovery plans based on participant strengths, values, and aspirations
  • Skill Building — Facilitating development of emotional regulation, coping strategies, and daily living skills
  • Trauma-Informed Support — Providing crisis intervention and de-escalation during periods of heightened distress
  • Community Integration — Supporting social connection, reducing isolation, and building meaningful community participation
  • System Navigation — Helping participants access and coordinate mental health services, NDIS supports, and mainstream programs
  • Transition Support — Providing stability during hospital discharge, housing changes, or other major life transitions
  • Empowerment & Advocacy — Building participant confidence in self-advocacy and decision-making
  • Collaborative Care — Working with multidisciplinary teams including psychologists, occupational therapists, and support workers
